I didn't know what was hanging on his back until I noticed it was similar to the thing on the wall in front of him which, in context, had to be a mirror.
Even when we know that, the actual joke isn't immediately obvious. It would probably be easier to get if the wife were smaller, as below.

I THINK it's implying that the man is sick and tired of being asked to move away from the mirror when shaving, every time his wife wants to use the mirror.
(She being a nagging, harridan wife, rather than a stupid, curvaceous one - the only two types available, apparently)
This is his makeshift solution.
The "that's fixed you!" look on his face swings it for me.
The rendering of the mirror is pretty hopeless, which doesn't help.

Shouldn't have his cock hanging out though.
